Care Management - A better way to deliver health care

Could coordinated health care improve outcomes for some of today's toughest public health issues? We think so

Up to 90,000. Down to 100. Up to 51 percent. They may seem like the ups and downs of the economy, but they’re actually the positive outcomes of Aurora Health Care’s Care Management initiatives. At Aurora, major public health issues get special attention every day. For people with acute episodes of illness, like a heart attack or pneumonia, we are implementing new treatment guidelines that are returning patients to health faster.

For people with chronic diseases, like asthma, diabetes and congestive heart failure, we are finding ways to help them better manage their health and improve their quality of life.

And the numbers are really adding up: Like lowering coronary artery disease patients LDL cholesterol to 100 to help reduce heart attacks through our nationally recognized cholesterol management initiative. Providing over 90,000 flu shots last year alone with the Shoo the Flu program. And, Aurora’s diabetes management initiative—where 51% of our diabetic patients surpass the national goal for hemoglobin A1C.

And this is just the beginning. Aurora Care Management brings together skilled professionals who have agreed to take on some of today’s biggest public health issues. Combining good science with common sense, yielding impressive results and improving the lives of their patients.

National recognition

Aurora Health Care has been selected as an honoree in the American Medical Group Association's Acclaim Award competition in recognition of its success in improving the care of patients with coronary artery disease. The Acclaim Award, now in its fourth year, recognizes health care organizations that have established innovative patient care processes and systems, and can measure and quantify improvements in outcomes for patients with specific diseases.
