Applications for Cardiovascular Disease fellow positions are accepted
through the National Resident Matching Program (NRMP), which uses the
computerized Electronic
Residency Application System (ERAS). Information about ERAS is
available through the Deans' Offices of all U.S. medical schools or from
the Educational Commission for Foreign Medical Graduates (ECFMG). We
accept ERAS applications from Jul. through Dec. 31 of each academic
year. Interviews are by invitation.

Fellowship Programs
To apply for an Aurora Interventional Cardiology & Electrophysiology
Fellowship program:
Attach a small photo to the application (mandatory).
Enclose a current curriculum vitae, medical school transcripts
or dean's letter/MSPE, copy of medical school diploma, USMLE
transcripts (1, 2, 3 steps)/FLEX/COMPLEX and copy of Internal
Medicine Residency certificate.
Enclose reference letters from at least three MD/DO clinicians
with whom you have done clinical work or rotations. If you are in a
training program, one letter must be from your program director.
If you have done research, enclose a reference letter from your
advisor and copies of abstracts or papers published.
If non-US citizen or international medical graduate, please
include a copy of your current J1 visa, permanent resident cared or
documentation (Aurora Health Care does not sponsor H category visa).
If you are a foreign medical graduate, enclose a photocopy of
your ECFMG certificate.
Enclose a two or three paragraph "Personal Statement" regarding
your goals during fellowship and future practice plans.
include a cover letter, signed and dated, attesting to the
information included as being accurate, to the best of your
knowledge.
Photocopy of your Cardiovascular Disease Fellowship and Internal
Medicine Residency programs certificates. They must be from an ACGME
accredited program. If you are currently in fellowship, the Program
Director's letter will be sufficient.
The Program Directors are: Masood Akhtar, MD, Clinical Cardiac
Electrophysiology Fellowship and Tanvir Bajwa, MD, Interventional
Cardiology Fellowship.
You may only apply to one advance fellowship program
(Interventional or Electrophysiology).

Applications are accepted from Jan. 1 through July 31.
Please note all materials must accompany your application packet, and
sent as one packet. This includes all letters of recommendations. The
only items we except outside the packet are your medical school
transcript and/or dean's letter/MPSE and official USMLE/FLEX/COMPLEX
transcript.
