Erika Aldag, Pharm.D., BCPS

Specialty Pharmacy Coordinator, Abdominal Transplant Erika received her Pharm.D. from Drake University. She completed her PGY1 Pharmacy Practice residency at the University of Nebraska Medical Center and herPGY2 Solid Organ Transplant residency at The Ohio State University Medical Center. She precepts both the PGY1 and PGY2 Critical Care Abdominal Transplant rotations.

Todd Armbruster, Pharm.D.

Clinical Pharmacist Todd graduated from Drake University. Todd practices and precepts in the intensive care unit at Aurora Sinai Medical Center.

Dareen Bleibel, Pharm.D.

Ambulatory clinical coordinator
Dareen received her B.S. in zoology and her Pharm.D. from the University of Wisconsin-Madison. She completed a PGY-1 Community Pharmacy Residency at Aurora HealthCare. She practices both at the Aurora St. Luke’s Outpatient Pharmacy and the Pharmacist-Managed Anticoagulation Clinic. She precepts the clinical services rotation.

Dennis Brierton, Pharm.D., BCPS, FASHP

System Director, Clinical Services Dennis received his B.S. from the University of Wisconsin and his Pharm.D. from the University of Illinois-Chicago. He completed a specialty residency in adult internal medicine at Purdue University.

Jane Bubik, Pharm.D.

Drug Information/Investigational Drugs Coordinator Jane received both her B.S. and Pharm.D. from Purdue University. She completed a PGY-1 Pharmacy Practice Residency at Rush-Presbyterian St. Luke’s Medical Center in Chicago. Jane co-precepts the drug use policy rotation and offers an elective investigational drugs rotation.

Matt Carleton, Pharm.D., BCPS

Specialty Pharmacy Coordinator, Drug Policy Matt received his Pharm.D. from Midwestern University in Glendale, AZ. He completed a PGY-1 Pharmacy Practice Residency at Aurora Health Care and continues to staff at St. Luke’s Medical Center. Matt co-precepts the drug policy rotation.

Margaret Cook, Pharm.D., BCPS

Specialty Pharmacy Coordinator, Infectious Disease Margaret received her B.S. from the University of Wisconsin and her Pharm.D. from the University of North Carolina. She completed a PGY-1 Pharmacy Practice Residency at Hartford Hospital, Harford, Connecticut. Margaret practices at Aurora St. Luke’s Medical Center. She co-precepts rotations in infectious diseases for PGY1 and PGY2 pharmacy residents and students.

Julie Dagam, Pharm.D., BCPS

PGY1 Residency Program Director - Aurora Health Care Metro, Inc Julie received both her B.S. and her Pharm.D. from the University of Wisconsin. She completed a PGY1 Pharmacy Practice Residency at Aurora Health Care, and currently practices at Aurora St. Luke’s Medical Center. Julie co-precepts the Epilepsy/Neurology elective learning experience (in the PGY1-Aurora Health Care Metro, Inc program) and the Pharmacy Recruiting required learning experience (in the PGY2 Health System Pharmacy Administration program). She also supports all of the residency programs at sites across the Aurora Health Care system.

Melissa Dahlgren, Pharm.D., BCPS

Specialty Pharmacy Coordinator, Medical and Surgical Pharmacy Services Melissa received her B.S. and Pharm.D. from the University of Minnesota. She completed a specialty residency in nephrology and renal transplant at the Hennepin County Medical Center in Minneapolis, Minnesota. Melissa practices at Aurora St. Luke’s Medical Center.

Chris Doede, PharmD, BCPS

Clinical Pharmacist Chris graduated from UW-Madison with a B.S in Biology and PharmD. He completed a PGY1 Residency at the University of Wisconsin Hospital. Chris currently precepts the cardiology rotation at St. Luke’s Medical Center.

Lynne Received her Pharm.D. from the University of Wisconsin and completed a PGY1 Pharmacy Practice Residency at Aurora Health Care. Since completion of her residency, Lynne has cared for patients with infections in a range of practice settings from ambulatory HIV care to serving as the pharmacy clinical coordinator for infectious diseases at Aurora St. Luke’s Medical Center. As part of her faculty appointment, Lynne practices as part of the ID Pharmacy team at Aurora St. Luke’s Medical Center two days per week.

Ashley Feldt, Pharm.D., M.B.A

Director of Pharmacy, Aurora St. Luke’s Medical Center Ashley received her Pharm.D. and M.B.A. from Drake University in Des Monies, Iowa. She completed her PGY-1 Pharmacy Practice Residency and PGY-2 Health-System Pharmacy Administration Residency at Aurora Health Care. She is a pharmacy manager at Aurora St. Luke’s Medical Center and oversees pharmacy services at the Aurora Cancer Care Clinics. She is involved in precepting the administration rotation.

Dave Galis, R.Ph.

Clinical Pharmacist Dave received his B.S. in Pharmacy from the University of Wisconsin. He practices in the Operating Room Satellite at Aurora St. Luke’s Medical Center and precepts the operating room rotation.

Craig Glienke, Pharm.D., BCPS

Clinical Pharmacist Craig graduated from the University of Minnesota, College of Pharmacy. He completed his PGY1 at Hennepin County Medical Center (HCMC) in Minneapolis, Minnesota. Craig precepts the PGY1 residents and PGY2 Critical Care resident in the Cardiovascular Intensive Care Unit.

Jon Godden, Pharm.D., BCPS

Specialty Pharmacy Coordinator, Cardiothoracic Transplant Jon received his Pharm.D. from the University of Iowa and completed his PGY-1 Pharmacy Practice residency at Aurora Health Care. He is now a specialty pharmacy coordinator in cardiothoracic transplant at Aurora St. Luke’s Medical Center. Jon precepts the advanced heart failure/cardiothoracic transplant rotation for the PGY-1 Pharmacy Residency Program, the PGY-2 Cardiology Pharmacy Residency Program and the PGY-2 Critical Care Pharmacy Residency Program.

Jennifer Godden, Pharm.D., BCOP

Specialty Pharmacy Coordinator, Oncology Jen received her Pharm.D. from the University of Iowa. She completed her PGY-1Pharmacy Practice and PGY-2 Oncology Specialty Residencies at Aurora Health Care. Jen serves as a preceptor for the PGY-1 outpatient oncology rotation and the PGY-2 oncology specialty residency.

Anne graduated with her Doctor of Pharmacy degree from Drake University in Des Moines, Iowa. She completed a Pharmacy Practice Residency with Barnes-Jewish Hospital in St. Louis, Missouri. Since completion of her residency, Anne has served as an inpatient clinical pharmacist and inpatient pharmacy clinical coordinator at Aurora Sinai Medical Center before joining the faculty at Concordia University Wisconsin School of Pharmacy. Anne maintains a clinical practice site at Aurora St. Luke’s Medical Center where she cares for geriatric, internal medicine, and general surgery inpatients and helps precept Aurora Health Care PGY-1 residents on the Medicine and Surgery rotations as well as Concordia University Wisconsin’s Doctor of Pharmacy students the Acute Care rotation.

Craig Grzendzielewski, Pharm.D., BCPS

Clinical Pharmacist Craig received his Pharm.D. from St. Louis College of Pharmacy. He completed a PGY1 Pharmacy Practice residentcy at Presence Saint Joseph Medical center in Joliet, Il. Craig currently precepts the PGY1 Cardiology rotation and co-precepts the PGY2 Cardiology - Heart Failure rotation at St. Luke’s Medical Center. Craig also serves as a primary preceptor for the Concordia University Wisconsin - School of Pharmacy acute care APPE rotation.

Tyler Guetschow, Pharm.D., BCPS

Clinical Pharmacist Tyler received his Pharm.D. from the University of Wisconsin-Madison School of Pharmacy. He completed his PGY1 Pharmacy Practice Residency and PGY2 Critical Care Specialty Residency at Aurora St. Luke’s Medical Center. Tyler is the preceptor for the Surgical ICU Rotation at St. Luke’s Medical Center.

Sr. Clinical Pharmacist Steve received his Pharm.D. from Creighton University in Omaha, Nebraska. He is the preceptor for the Neonatal Intensive Care Unit/Women's Care rotation at Aurora Sinai Medical Center.

Emma Hews, Pharm.D

Director of Pharmacy, Aurora Sinai Medical Center Emma received her BS in Biology/Biotechnology from Worcester Polytechnic Institute and her Pharm.D. from Shenandoah University. She completed both the PGY-1 Pharmacy Practice Residency and the PGY-2 Health-System Pharmacy Administration Residency at Aurora Health Care.

Stacy Hilgendorf, Pharm.D., BCPS

Clinical Pharmacist Stacy received her Pharm.D. from the University of Wisconsin and completed her PGY1 Pharmacy Practice Residency at Aurora Health Care. She is a preceptor for the PGY1 Adult Medicine Rotation and has pharmacy experience staffing oncology, general medicine and critical care at St. Luke’s Medical Center.

Senior Director of Pharmacy Operations Arlene received both her B.S. in Pharmacy and her M.S. in Hospital Pharmacy Administration from the University of Wisconsin. She completed a PGY-1 Pharmacy Practice Residency and a PGY-2 Pharmacy Administration Residency at the University of Wisconsin Hospital and Clinics.

Kelcy Johnson, Pharm.D., BCPS

Clinical Pharmacist Kelcy received her B.S. in Biology from the University of Notre Dame and Pharm.D. from the University of Wisconsin – Madison. She completed her PGY1 with Aurora Health Care. She currently precepts for the Adult Medicine rotation at St. Luke’s Medical Center.

Julie L. Kindsfater, Pharm.D., BCPS

Drug Policy, Formulary Coordinator, Sr. Julie received her Pharm.D. from the University of Minnesota – Twin Cities. She completed a PGY-1 Pharmacy Practice Residency at Froedtert Memorial Lutheran Hospital in Milwaukee. Julie co-precepts drug policy rotation. She chairs the Aurora Medication Safety Committee and is pursuing a Masters in Patient Safety Leadership through the College of Medicine at the University of Illinois – Chicago.

Al Klewin, R.Ph.

Regional Coordinator, Pharmacy Care Management Al received his B.S. in Pharmacy from the University of Wisconsin. He has done extensive work with Medication Safety and Patient Safety within Aurora Health Care and with several community patient safety projects. Al co-precepts the PGY-1 Pharmacy Practice Resident Medication Safety learning experience for the Mid-Market. Aurora St. Luke’s Medical Center is his primary practice site. He also coordinates physician clinical adoption, transformation and clinical analytics in CPOE for the Aurora Health Care System.

Chris Klink, Pharm.D., BCPS

Clinical Pharmacist Senior Chris received his B.S. in pharmacy from the University of Wisconsin at Madison, and his Pharm.D. from the University of Arkansas for Medical Sciences. Chris precepts the Family Practice rotation. He practices at the St. Luke’s Family Practice Center and the Aurora Pharmacy in the Aurora St. Luke’s Physician Office Building.

Christopher Knock, Pharm.D.

Clinical Pharmacist Christopher received his Pharm.D. from Drake University. He is the preceptor for the Emergency Medicine rotation for the PGY-1 Pharmacy and PGY-2 Critical Care Residency Programs at Aurora St. Luke’s Medical Center.

Sr. Director of Pharmacy Operations Al received his B.S. in Pharmacy from Purdue University. He obtained his M.S. in Hospital Pharmacy Administration from the University of Wisconsin and completed a two-year residency in practice management at the University of Wisconsin Hospital and Clinics.

Tracy J. Mathia, Pharm.D., BCPS

Sr. Clinical Pharmacist Tracy received her Pharm.D. from Creighton University in Omaha, Nebraska. She completed a PGY-1 Pharmacy Practice Residency with Aurora Health Care. Since completion of her residency, Tracy has served as an inpatient clinical pharmacist, staffing primarily in cardiac units, and as specialty pharmacy coordinator in cardiothoracic transplant at ASLMC before joining the team at the Packaging Center. Tracy is the coordinator for the Clinical Forum CORE learning experience for the PGY-1 Metro Pharmacy Practice Residency class.

Michelle Maynard, Pharm.D., BCPS

Sr. Clinical Pharmacist Michelle received her Pharm.D. from the University of Iowa. She completed a PGY-1 Pharmacy Practice Residency at Aurora Health Care. She co-precepts the PGY-1Epilepsy/Neurology learning experience, as well as the PGY-2 Neuro ICU rotation for the specialty residency in critical care.

Barry received his B.S. in Pharmaceutical Sciences and Pharm.D. from The Ohio State University and his M.S. in Health-System Pharmacy Administration from the University of Wisconsin. He completed his PGY-1 Pharmacy Practice Residency and PGY-2 Pharmacy Administration Residency at the University of Wisconsin Hospital and Clinics. In addition to being the PGY-2 Pharmacy Informatics Program Director, he is a preceptor for the PGY-2 Informatics Residency, PGY-2 Health System Pharmacy Administration Residency, and PGY-1 Pharmacy Residency.

Michael A. Metz, R.Ph.

Director of Pharmacy, Aurora St. Luke’s Medical Center Mike received his B.S. in Pharmacy from the University of Wisconsin. Mike oversees Central Pharmacy operations, automation and sterile products for Aurora St. Luke’s Medical Center. Mike precepts the administration rotation for PGY-1 residents and co-precepts PGY-2 administration residents for operation rotations at Aurora St. Luke’s Medical Center.

Valerie co-precepts both the PGY-1 and PGY-2 infectious diseases rotations.

Suzie Robison-Strane, R.Ph., BCPS

Sr. Clinical Pharmacist Suzie received her B.S. in Pharmacy and completed a residency in nuclear pharmacy at the University of Utah. She practices at the Aurora Pharmacy Central Order Verification Department, is chairperson for the multidisciplinary ASLMC/ASMC MedSafe Committee and co-precepts the ASLMC PGY-1 Pharmacy Resident Medication Safety learning experience.

Ryan Servais, Pharm.D., BCPS

Clinical Pharmacist Ryan received his Pharm.D. from the University of Wisconsin. He completed his PGY-1 Pharmacy and PGY-2 Critical Care Residencies at Aurora Health Care. Ryan precepts the CVICU rotation and also leads topic discussions as part of the critical care, cardiology, ED and OR rotations.

Stephanie Shuey, Pharm.D.

Clinical Pharmacist Stephanie received her Pharm.D. from the University of Pittsburg. She completed her PGY1 Pharmacy Practice Residency and PGY2 Oncology Pharmacy Residency at Aurora St. Luke’s Medical Center. Stephanie currently precepts the PGY1 inpatient oncology rotation.

Chad Smith, Pharm.D., MBA, BCPS

Director of Pharmacy, Aurora St. Luke’s Medical Center Chad received his BS from Marquette University, and his Pharm.D. and MBA from Creighton University. He did a PGY-1 Pharmacy Practice residency and a specialty PGY-2 Health System Administration residency at St. Luke’s Medical Center, Aurora Health Care. Chad precepts the administration rotation for PGY-1 residents and co-precepts PGY-2 administration residents for operation rotations at Aurora St. Luke’s Medical Center.

Theresa Smith, PharmD, BCPS

Pharmacy Manager, Centralized Order Verification Theresa received both her B.S. in Health Administration and Policy and Pharm.D. from Creighton University in Omaha, Nebraska. She completed her PGY-1 Pharmacy Practice Residency and PGY-2 Health-System Pharmacy Administration Residency at Aurora Health Care. Currently, she is the pharmacy manager for the Centralized Order Verification team and has been involved as a preceptor for clinical forum and resident year-long projects.

Frank Spexarth, R.Ph., BCPS

Specialty Pharmacy Coordinator, Cardiovascular Surgery Pharmacy Services Frank received his B.S. in Pharmacy from the University of Wisconsin. He practices at Aurora St. Luke’s Medical Center and is an ACLS instructor. He co-precepts cardiology, cardiovascular surgery and critical care rotations.

Peter Stuessy, Pharm.D., BCPS

Sr. Clinical Pharmacist Peter received his Pharm.D. from the University of Wisconsin-Madison. He completed his PGY-1 Pharmacy Residency at Aurora St. Luke’s Medical Center. He precepts the PGY-1 Pharmacy Oncology Ambulatory Care rotation and is a preceptor in training for the PGY-2 Specialty Oncology Solid Tumor rotation.

Ryan Van Engel, Pharm.D.

He completed his PGY1 Pharmacy Practice Residency a t the University of Iowa Hospitals and Clinics and then completed his PGY2 Critical Care Specialty Residency through Midwestern University, practicing at Northwestern Memorial Hospital. At both programs he obtained a teaching certificate. Ryan precepts the PGY1 residents on 8 center and for both PGY1 and PGY2 residents on the SICU rotation.

Angela Volquardsen, Pharm.D., BCPS, CACP

Clinical Pharmacist Angela received her Pharm.D. from the University of Kansas and completed a PGY-1 Pharmacy Practice Residency at Aurora Health Care. Angela has continued involvement with both the Adult Medicine rotation at Aurora St. Luke’s Medical Center and the Clinical Teaching rotation.

Angie Weitendorf, PharmD, BCPS

Specialty Pharmacy Coordinator, System Pharmacy Angie received her Pharm.D. with a concentration in diabetes from Drake University in Des Moines, Iowa. She completed her PGY-1 Pharmacy Practice Residency and PGY-2 Health-System Pharmacy Administration Residency at Aurora Health Care. Angie is a system pharmacy coordinator and helps precept the Medicine rotation based out of St. Luke’s and is a preceptor for the clinical teaching rotation for the PGY1 Pharmacy Residency. Angie also precepts the PGY2 Oncology Pharmacy Resident Medication Safety learning experience.

Paul Windisch, Pharm.D.

Specialty Pharmacy Coordinator, Drug Policy Paul received his B.S. in Economics & Statistics from University of Minnesota and his Pharm.D. from the University of Nebraska Medical Center. He completed a PGY-1 Pharmacy Practice Residency at University of Wisconsin Hospital & Clinics, and completed a two-year fellowship in pharmacoeconomics, technology assessment & outcomes research at University Health System Consortium (UHC). Paul co-precepts the drug policy rotation.

Prati Wojtal, R.Ph., M.S.

Director of Ambulatory Pharmacy Services Prati received her B.S. in Pharmacy and M.S. in Hospital Pharmacy Administration from the University of Minnesota. She completed a two-year administrative residency at United and Children's Hospitals in St. Paul, Minnesota. Prati has been very active with the state pharmacy organizations serving first as secretary then as president for the Wisconsin Society of Health-System Pharmacists in 1995-96, and also board member for the Pharmacy Society of Wisconsin in 1998-99 and again in 2006 – 2008. She was appointed to the ASHP Council on Organizational Affairs and the Council on Legal and Professional Affairs, invited to participate as a member of the Consensus Recommendations Panel at the 2014 ASHP Ambulatory Care Conference and Summit, and recognized as a Fellow of ASHP in June 2014

He completed a two-year residency in practice management at the University of Wisconsin Hospital and Clinics. Tom has served as a member of the ASHP Commission on Credentialing, the body responsible for accreditation of pharmacy residency programs.

Stacy Wucherer, R.Ph.

Manager of Packaging Center Pharmacy Services Stacy received her B.S. in Pharmacy from the University of Wisconsin. She completed the PGY-1 Pharmacy Practice Residency and the PGY-2 Health-System Pharmacy Administration Residency at Aurora Health Care. Stacy is responsible for the packaging center and pharmacy automation.

Sol Yoder, Pharm.D., BCOP

Jeff precepts the Medical Respiratory Intensive Care Unit (MRICU) rotation for the PGY1 Pharmacy Practice residents, the Aurora Medical Group Intensivist rotation and the MRICU rotation for the PGY2 Critical Care resident as well as the Oncologic Critical Care rotation for the PGY Oncology residency.

Nathaniel Zook, Pharm.D., BCPS

Clinical Pharmacist Nate received his Pharm.D. from Philadelphia College of Pharmacy within the University of the Sciences. He completed his PGY-1 Pharmacy Residency at Penn State Milton S. Hershey Medical Center and his PGY-2 Critical Care Specialty Residency at Orlando Regional Medical Center. Nate currently precepts for the PGY-1 and PGY-2 Critical Care MRICU rotation at St. Luke’s Medical Center.

Sarah Zukkoor, Pharm.D., BCPS

Specialty Pharmacy Coordinator, Cardiology Sarah received her Bachelors of Science in Human Biology from Michigan State University and her Pharm.D. from the University of Michigan, College of Pharmacy. She completed her PGY1 Pharmacy Residency at the University of Toledo Medical Center and PGY2 Cardiology Pharmacy Residency at the Ohio State University Medical Center. Sarah is a preceptor for acute care cardiology and the Coronary Intensive Care Unit (CICU) rotations at ASLMC.

Nick Zupec, PharmD, BCPS

Clinical Pharmacist Nick received his BS in biomedical sciences from Marquette University and his PharmD from the University of Illinois at Chicago. He completed a PGY1 pharmacy practice residency at Aurora St. Luke’s Medical Center and a PGY2 pharmacy residency in internal medical at the Zablocki VA Medical Center. Nick works on the general medicine and cardiology floors and co-precepts the medicine, cardiology, and clinical teaching rotations.
