The deadline for application materials was November 30, 2007. To submit an application for the class beginning in September 2009, please refer to the guidelines below. Application overviewApplication forms for 2009 will be available from October 1, 2008 - November 23, 2008. All applications must be postmarked by November 28, 2008. No applications will be excepted if postmarked after this date. A $25.00 non-refundable application fee is required. All payments should be made by personal check or money order payable to Aurora St. Luke's Medical Center. Application packets must be mailed to:
Faxed, emailed, or hand delivered applications will not be accepted. Instructions on submitting an applicationPlease note, class size is limited and not all applicants are selected for admission to the program. Applicants who are selected for interviews will be notified by mail. All program pre-requisites must be completed before an application is submitted. Allied Health Students must successfully complete their program prior to the start of class in September. Failure to meet any of the application requirements will result in loss of application eligibility. All applicants must submit a completed application form along with the $25.00 application fee. Applicants must complete the Drug-screening consent form. Applicants refusing to consent and submit to a complete drug screening will not be considered for acceptance into the program. Applicants must provide a brief autobiography, 1-2 typed pages, covering the past four years of the applicant's life. The narrative must also state why he/she has an interest in the field of sonography and Aurora St. Luke's Medical Center School of Diagnostic Medical Sonography. Applications must have three (3) signed letters of professional reference along with the applicant recommendation form. (Original letter signed & dated within 6 months of the application date). Applications with less than three letters of reference will not be considered. Applicants must submit documentation showing recent job shadowing experience for a minimum of eight (8) hours at Aurora St. Luke's Medical Center. (Refer to program prerequisites for detailed information). Documentation of patient care experience is required for all applicants applying under prerequisite #2. (Refer to program prerequisites for detailed documentation requirements). Submitting transcriptsOfficial Academic transcripts from high school and post-secondary educational institutions are required to document the completion of pre-requisites. To maintain eligibility, official sealed academic transcripts must be received and postmarked by application deadline. Transcripts should be sent to the program directly from the educational institution. Hand carried transcripts will not be accepted. Student selectionThe school does not maintain a waiting list; rather an Admissions Committee selects the most qualified applicants on an annual basis. Applicant/candidate assessmentAll application elements are reviewed and scored. Applicants meeting the required minimum point value are invited for a personal interview with the Admissions Committee. Personal interviews are typically scheduled from March through April. Please note that not all applicants will receive an interview. The decision of the committee is based upon information provided in the application and the personal interview. Factors considered are:
Candidate notificationThe Admissions Committee will notify all applicants of their final decision in writing in May. The candidates who are offered positions must submit a written letter of acceptance and a non-refundable $500.00 acceptance fee. The acceptance fee is applied to the tuition. General eligibility requirementsAll required information must be submitted by the stated due-dates. Failure to meet all deadlines will result in a rejection of the application. In addition, official transcripts must verify all educational information provided. Any discrepancies will terminate an applicant's eligibility. As a condition of admission, applicants considered for an appointment will be required to pass a drug screen and physical examination. The cost of said tests are the responsibility of the sponsoring institution. Applicants must have foreign transcripts evaluated for equivalency in the Unite States. Applicants with no additional education obtained in the U.S. must provide proof of academic ability, such as official ACT or SAT results, dated within the last three years.
