Bequest wording samples

A bequest for any of the programs and services of the hospital should be made to Aurora St. Luke's Medical Center. Staff of the Philanthropy Office are available to discuss bequest forms or other estate planning matters with your attorney, who may contact the Philanthropy Office without revealing your name, if you so desire. All discussions are strictly confidential.

The proper wording for a bequest to Aurora St. Luke's Medical Center is important to ensure your wishes will be followed. The following are some suggested forms you and your attorney may consider in naming Aurora St. Luke's Medical Center in your will.

General unrestricted bequest

"I give and bequeath to St. Luke's Medical Center, Milwaukee, Wisconsin, a nonprofit corporation, the sum of $ ______ (OR a percentage of _______ OR the securities or property described herein, namely …) to be used as directed by the Board of Directors of St. Luke's Medical Center, Milwaukee, Wisconsin."

Restricted bequest

"I give and bequeath to St. Luke's Medical Center, Milwaukee, Wisconsin, a nonprofit corporation, the sum of $ ______ (OR a percentage of ________ OR the securities of property described herein, namely …) and request same be used as directed by the Board of Directors of St. Luke's Medical Center, Milwaukee, Wisconsin for … (describe the specific program bequest is to be used for)."

Residuary bequest

"All the rest, residue and reminder of my estate, wherever situation, I give, devise and bequeath to St. Luke's Medical Center, Milwaukee, Wisconsin, a nonprofit corporation, to be used … (describe use ad indicated in the suggested forms above)."

Any of the above suggested forms for bequests can be made through the preparation of a codicil to your present will. Please consult your attorney to determine if this is appropriate for your particular situation.
