To contact the chaplain, dial extension 5513 or dial "O' for the Medical Center operator. A chapel service is held on the first Tuesday of each month. You may watch the chapel service on Tuesday and Thursday mornings at 11:00 on Channel 7. Area church service tapes are shown on Sundays. Discharge planning and case management Discharge planning actually begins at the time you enter the Medical Center. A case manager will help you and your family plan your care during and after hospitalization. The case managers can help you obtain:
Your nurse can help you contact your case manager. Your doctor will decide when you are well enough to go home. Your case manager and nurse will help you make arrangements. Checkout time is 11 a.m. We will give you instructions about the care you need after you leave, including medications, treatment and follow-up visits to your doctor. Check all areas of your room to make sure you have all your belongings. Ask your nurse to help you get any medications and valuables stored here. Aurora Sheboygan Memorial Gift Shop is located on 1st floor at the visitor entrance. Hours: Monday through Friday, 9 a.m. to 4 p.m. Hours: Monday through Friday, 7 a.m. to 4:30 p.m. After signing a consent form, you or your authorized representative may read your medical record. You may also get a copy of your x-ray report or have your x-rays sent to another health care provider. Your request may take 24 hours to complete. A fee is charged to cover the cost of copies. All mail, flowers and gifts coming to the Medical Center are promptly delivered to you. If you have already left, the items will be re-addressed and forwarded to your home. Nutrition Service staff will ask you to select your meals according to the diet your doctor ordered. Please let them know if you have any specific food concerns. Ask to speak with a registered dietitian if you have any questions about your diet. You and your family are invited to use the Aurora Sheboygan Memorial Medical Center's Eliza Prange medical library. The library is located near the visitor entrance and gift shop. An information sheet in your patient folder explains how to access health information from the library and other sources. Hours: Monday through Friday, 8 a.m. to 8 p.m. Patients at Aurora Sheboygan Memorial Medical Center may receive a free copy of "The Sheboygan Press" on weekday afternoons. The "Milwaukee Journal Sentinel" and "USA Today" papers are also available weekday mornings in the lounges. If you have any concerns or complaints about your care, please talk to your nurse. If you feel your concern is not resolved, ask to speak with the Department Manager. If the Department Manager is not able to resolve your concern, the Medical Center Administrator will be notified and will contact you directly. You may also write to the Medical Center Administrator. We will acknowledge your written concern and review it within 7 business days. If you are not satisfied with the action taken to resolve your concerns, you may contact the Bureau of Quality Assurance, 1 West Wilson Street, P.O. Box 2969, Madison, WI, 53701-2969. (608-266-8084). After you go home, you may receive a written or telephone satisfaction survey. This gives you the chance to make any comments, suggestions or address any problems you experienced. The information you give us in the survey helps us to improve our services. The Medical Center is smoke-free. Talk with your doctor or nurse if you are concerned about nicotine withdrawal. We suggest that you leave valuables at home or send them home with a family member or friend. Have your nurse store your valuables in the Medical Center safe if you cannot send them home. The Medical Center is not responsible for any valuables not placed in the safe.
You may use your own small personal electrical appliances such as hair dryers, curling irons and shavers. Devices must be in good working order with no damaged cords. The Plant Operations department must check all other electrical devices before use. Please talk to your nurse about appliance use when you are admitted. There are no set visiting hours, unless your medical condition requires visiting restrictions. Special visiting hours apply for patients in Behavioral Health units at Aurora Sheboygan Memorial. Ask your nurse for details. Visitors with colds, flu, or contagious illnesses should not visit. Food and soft drinks are available 24 hours a day through vending machines and microwaves located in the cafeteria area at Sheboygan Memorial. Sheboygan Memorial Cafeteria Guest trays are also available for family members who wish to dine with you in your room during regular patient meal times. Order and pay for guest trays through the nutrition services representative. A volunteer patient advocate may visit you during your stay. The volunteer patient advocate will ask you questions about your stay. Feel free to voice your opinions or concerns, good or bad. Your comments will be passed on to the appropriate persons. Nurse Call When you push the button at your bedside, our nurse call operator answers your call. She sends your request to the proper staff member. Television A color television is mounted on a "gravity free" arm in your room for easy adjustment. A remote control is available. Channel 7 at Aurora Sheboygan Memorial Medical Center is our own station. Channel 7 broadcasts chapel services, area church services, relaxation tapes, and a variety of health and disease education videos. Your admission folder contains the Channel 7 schedule. You may also request the health videos listed in the program guide to be broadcast at a special time. Channel 24 is "The Patient Channel." GE Medical Systems operates The Patient Channel, which has a wide variety of up-to-date health education topics. Channel 8 broadcasts WHBL radio. Community members are welcome to visit our libraries and use our print and online resources as well. Photocopies are 10 cents per page. Books and audiovisuals may be checked out for 2 weeks.
