Aurora Health CenterDiabetes self management programA comprehensive approach to diabetes careOur program offers you and your family consistent teaching, coordinated clinical follow up and support through the long-term management of your diabetes. Our diabetes education team includes a registered nurse and registered dietitian. We view diabetes as an ongoing process, which requires the commitment of everybody involved, including you. To help, we provide the support and encouragement you need to take an active role in your own diabetes care. We'll work with you to develop an appropriate plan for your own best care. Education and nutrition counselingThese are important components of our program. Educators will provide the information you need to adapt your diabetes management skills to your lifestyle, and adjust the plan as your needs change. Our team will provide information you need to manage your diabetes care effectively. We'll prepare you to make informed decisions about diabetes self-management, and to define realistic goals in your efforts to life with and manage diabetes. Ask your doctor for a referralYour doctor can make the referral and a schedule will call to arrange an appointment for you. Diabetes support groupsOur diabetes support groups are an extension of our team approach to diabetes management. The group setting provides opportunities for sharing and discussion, tips for living with diabetes, information on current trends in diabetes management and guest speakers on topics related to diabetes.
