Nurses collaborate with a supportive team of professionals while working one-on-one with their patients to help them achieve a very important goal, staying independent in their own home.

Pediatric Care CoordinatorImagine: Utilizing and challenging your nursing skills in a community setting

We are looking for a RN to be responsible for initiating the admission process for VNA services in Kenosha.

  • Attends discharge planning rounds on the individual hospital units
  • Works with hospital staff regarding referrals to the VNA
  • Collects and records medical, social and reimbursement data from the hospital pertinent to initiating home care services
  • Obtains and assesses information from patients, families, hospital personnel and physicians relevant to patient admission
  • Actively plans home care, hospice, DME/RT and/or IV services with the physician, patient and family, as well as any other disciplines who are involved with the patient's care
  • Interprets and verifies reimbursement criteria for VNA services
  • Works with family on assessment for financial assistance prior to hospital discharge.

Technical skills and knowledge needed

  • Current licensure as a Registered Nurse (RN) granted by the State of Wisconsin
  • Minimum of two years nursing experience required
  • Home health care, discharge planning or case management experience strongly desired
