Accreditation statement

The Accreditation Statement is the standard statement that must be used by all accredited institutions and organizations. There are two different statements (directly sponsored or jointly sponsored) that might be used depending on the relationships of the organizations involved in planning and implementing the activity.

The accreditation statements must appear on all CME activity promotional materials and brochures. However, the accreditation statements do not need to be included on initial, save-the-date type activity announcements. Such announcements contain only general, preliminary information about the activity, such as, date, location, and title. If more specific information is included, like faculty and objectives, the accreditation statements must be included.

The Wisconsin Medical Society also mandates that the AMA/PRA credit Designation Statement appear on promotional materials and brochures separately from the statement of accreditation.

For a directly sponsored activity

An activity planned and implemented by Aurora Health Care, (the sponsor) and/or a physician/department/facility within the Aurora system.

The Aurora Health Care is accredited by the Wisconsin Medical Society to provide continuing medical education for physicians.

Designation Statement (do not include this title when using statement in printed material)

Aurora Health Care designates this [insert learning format] for a maximum of [enter in number of credits] AMA PRA Category 1 Credit(s). Physicians should claim only the credit commensurate with the extent of their participation in the activity.

For jointly sponsored activity

An activity planned and implemented by one accredited provider (i.e., Aurora Health Care) working in partnership with a non-accredited, non-Aurora entity (i.e., the Wisconsin Heart Society). The accredited provider must ensure compliance with the Wisconsin Medical Society's Essential Areas, Elements and Policies and therefore take responsibility for the activity as indicated in the accreditation statement.

Statement Of Accreditation Jointly sponsored activity (do not include this title when using statement in printed material)

This activity has been planned and implemented in accordance with the Essential Areas, Elements and Policies of the Wisconsin Medical Society through the joint sponsorship of Aurora Health Care and [name of non-accredited provider]. The Aurora Health Care is accredited by the Wisconsin Medical Society to provide continuing medical education for physicians.
