Geriatric behavioral health servicesThe Geriatric Behavioral Health Program provides highly specialized care to older adults and their families. As an integral part of an innovative health care delivery system, geriatric behavioral health services are offered in collaboration with a variety of health care practitioners including physicians, nurses, social workers, occupational therapists and chaplains. Based in comprehensive medical centers, the geriatric programs at Aurora Sinai Medical Center and St. Luke's South Shore are attentive to both the physical and emotional needs of the older adult, offering both outpatient and inpatient care. At both locations, services are provided by a multidisciplinary treatment team that assesses the individual needs of the patient and develops a plan of care. Family and caretaker involvement is encouraged as treatment goals are developed and monitored. As needed, community resources are also identified and included in the patient's care plan. Resources may include residential options, financial or social supports, or in-home care services. Contact information
