Heart Failure Case Management

The goal of this service is to assist you in managing your heart failure at home. Education about how to live well with heart failure will be an important focus of our efforts.

These case management services are available to those Aurora heart failure patients who have unmet needs. This free service is provided by an expert heart failure nurse via telephone. Services include:

• Education
• Resource referral
• Distribution of written materials and tools
• Follow up

All medical information is kept confidential and may be released only to program staff, your doctor or your health plan.

A family member or friend can be helpful as you learn to take better care of your health. Patients are encouraged to share program materials with anyone who is involved in their care.
