The hospice team 
The Aurora Visiting Nurse Association Hospice team is an interdisciplinary team, including:
Patient's personal physician
- Continues to oversee medical care with the help of the hospice team Hospice medical director
- Advises the team and works with the patient's physician regarding plan of care
Nurse
- Coordinates all care provided for the patient
- Experienced in pain control, symptom management, counseling, teaching and support
Social worker
- Provides emotional support for the patient and family members
Chaplain
- Coordinates overall spiritual care according to wishes of the family and provides comfort, counsel, and support for the patient and family
Home health aide
- Assists with personal care for the patient
Pharmacist
- Provides consultation regarding any medication issues 24 hours a day
Dietitian
- Provides expertise in nutritional needs of terminally ill patients
Therapists
- Provide occupational, physical, and speech-language therapy as needed
Bereavement counselor
- Provides bereavement support and counseling
Volunteers
- Listen and help the patient and family, provide companionship, do shopping and errands and provide relief for the primary caregivers
A team member is on-call 24-hours a day to answer questions, discuss concerns and visit patients when needed.
