Referrals & fees

Please ask your doctor to refer you to a dietitian. Your doctor can simply fill out an order form and fax it to the desired location listed on the bottom of the form. The form is an Adobe Acrobat file. If you need to download the viewer you may do so by clicking here.

A doctor's order is required for most locations.

Aurora Sheboygan Memorial Medical Center accepts self-referrals for weight management. Aurora Sinai Medical Center accepts self-referrals for weight management and sports nutrition.
