Crossbow evaluation

The State of Wisconsin Department of Natural Resources (DNR) has available a permit to hunt with a crossbow. In order for an applicant to qualify for the permit, he/she must demonstrate weakness, decreased motion or coordination restricting the individual's ability to use conventional archery equipment. In Wisconsin, Registered Occupational Therapists and Physical Therapists are licensed to complete this assessment on orders from a physician. The Occupational Therapists at the Aurora Rehabilitation Center are experienced in the completion of crossbow evaluations.

How to get a permit to hunt with a crossbow

  • Obtain an application form from any DNR office
  • Page one of the application needs to be completed by the applicant and a physician
  • Obtain an order from the physician for an Occupational Therapist to complete a crossbow evaluation if you potentially qualify for the permit due to a permanent substantial loss of function or rage of motion in one or both arms, hands, or shoulders.
  • Call to schedule the evaluation. The crossbow evaluation may be scheduled at the Aurora Rehabilitation Center at 262-670-4300, or at the Aurora Rehabilitation Center in West Bend at 262-306-6100. The assessment takes about 30 minutes
  • Attach the completed crossbow evaluation to the application when submitting it to the DNR for approval
