Rehabilitation day program

We have expanded our inpatient rehabilitation into an outpatient program where the patient and family can receive the same comprehensive care at home.

Along with requiring interdisciplinary care, appropriate patients for this program are medically stable but may benefit from some medical monitoring. Patients should not require skilled nursing at night and will need to have a home environment and family support system capable of safely meeting their night and weekend care needs. 

The rehabilitation day program is Monday through Friday, with patients typically beginning their therapy 5 times per week, 3-6 hours per day. As functional gains are made, the schedule may decrease to 3 days, or from full days to half days. Restoration of the patient's function will allow a transfer to the next, less intensive level of rehabilitation care, such as outpatient therapy.

Experienced staff

The Rehabilitation Day Program consists of a team of professionals from physiatry, physical therapy, occupational therapy, speech pathology, recreational therapy, social services, psychology and nursing. During the patient's course of treatment, the team meets on a regular basis with the patient and family to discuss progress and future strategies of care.

Individualized treatment

The team evaluates the patient's needs and abilities to establish their treatment plan. Individualized patient goals are established involving each of the following areas:

  • To challenge each person to reach their best physical, emotional, reasoning, social and work potential through a team treatment approach
  • To educate the person and family about rehabilitation and how each team member facilitates the treatment process
  • To assist the patient and family with adjusting to a new lifestyle
  • To maintain or improve the patient's level of independence after discharge through the use of community resources 

Comprehensive care provided

The comprehensive treatment program provided by our rehabilitation team consists of 1-on-1 and group sessions which emphasize the following areas:

  • Functional mobility and transfer training
  • Education on skin care
  • Gait training
  • Balance
  • Safety
  • Dietary considerations
  • Visual-perceptual skills
  • Self-care abilities/ADLs
  • Home management skills
  • Community re-integration
  • Communication and cognition
    (expressive skills, receptive skills, decision making and problem solving, safety/judgement)
  • Fine motor skills
  • Various support groups are also available to our patients and their families
