Rehabilitation day program

Rehabilitation Day Program At the Aurora Rehabilitation Center, we strive to meet the changing needs of our patients and their families, providing the best care at the best time and location. Our current inpatient rehabilitation programs provide exceptional therapy services to our patients who have suffered physical limitations caused by stroke, neurological disorders, amputation, and others. These services have been expanded into an outpatient Rehabilitation Day Program where the patient and family can receive an intense level of care, along with the benefits of living at home versus in the hospital.

The Rehabilitation Day Program consists of a team of experienced professionals from physiatry, physical therapy, occupational therapy, speech pathology, therapeutic recreation, and psychology. In addition, other professionals from nursing and social work may be accessed as appropriate. During the course of treatment, the team will meet on a regular basis with the patient and family to discuss progress and future strategies of care. Appropriate patients for this program are those who require interdisciplinary care and are medically stable, but may benefit from some medical monitoring. Patients should not require skilled nursing at night and will need to have a home environment and family support system capable of safely meeting their night time and weekend care needs,as well as providing transportation to and from therapies. Information about Transit Plus, a community based transportation service, is available upon request.

The Rehabilitation Day Program is different than a traditional outpatient therapy experience because it is designed to provide treatment by multiple disciplines with a schedule that can be expanded to meet the individual needs of the patient. Initially, the frequency of treatment is more intense, then moves to fewer times per week in order to spread the sessions over a longer period of time for maximum benefit. When indicated, a patient may stay beyond the assigned therapy hours in order to benefit from rest breaks, including the lunch hour, and to work on exercise programs or other activities with the supervision of the rehabilitation aide. We are also able to offer the services of a therapeutic recreation specialist who will focus on the recovery of leisure skills. Aquatic therapy is also available at our on-site pool.

Individualized treatment

The team of rehabilitation specialists evaluates the patient’s needs and abilities in order to establish their treatment plan. Individualized patient goals are established:

  • To challenge each person to reach their best physical, emotional, speech, social and work potential through a team approach
  • To educate the person and family about rehabilitation and how each team member facilitates the treatment process
  • To assist the patient and family with adjusting to a new lifestyle
  • To maintain or improve the patient’s level of independence after discharge through the use of community resources

Comprehensive Care Provided

The comprehensive treatment program provided by our rehab team consists of sessions that emphasize:

  • Safety
  • Balance
  • Self-Care Abilities/Home Management
  • Communication and Cognition
  • Functional Mobility and Transfer Training, including Gait Training
  • Low Vision/Visual-Perceptual Skills
  • Fine Motor Skills
  • Community Re-integration Skills
  • Home Program Instruction

Information about various support groups and peer visits is also available to our patients and their families.
