Sports medicine

2008 Pre-Season Student Sports Physicals

Sports physicalsPhysicals (approximately an hour) include:

  • Medical exam performed by a physician, nurse practitioner (NP) and/or physician assistant (PA). Sports participation clearance provided by doctor (MD), advanced practice nurse prescriber (APNP) or doctor of osteopathic medicine (DO).
  • Joint/muscle/ligament screening
  • Injury prevention information

Register now!  Walk-in appointments are not guaranteed.

  • Physicals meet school and WIAA requirements, but should not replace well-child exams.
  • Parent/guardian must accompany the athlete or provide a signed parental consent form
  • Students should wear shorts and gym shoes

Physicals are $35 per student. Family discount: Two physicals for $60; three physicals for $80.

Family discount applies to related family members; physicals to be performed at the same location and date. Payment by cash or check is due at the time of service. 

If a student athlete is in need of physical appointment after the dates listed, please contact your school’s athletic trainer for help in locating a physician/facility.
