Nursing and MAPrecepted Student Placement

Request a precepted student placement.

Thank you for considering Aurora Health Care for your precepted student placement. We are proud to sponsor nursing and medical assistant clinical preceptorships at locations across Wisconsin. Please submit your request below. All requests will be reviewed by Aurora Clinical Placement Specialists and site leadership; submission of a clinical request does not guarantee placement.

Steps for School Placement Coordinators

Precepted Student Request Form for School Placement Coordinators

Only the school clinical placement coordinator should complete this request form.

Review and complete the following:

  • Step 1: Ensure a school affiliation agreement is in place. If there is not an agreement in place, please contact either: NOTE: new agreements may take up to 3 months to get into place.
  • Step 2: You will need the following data to complete the request form:
    • Start and end date of the clinical
    • Student name (last, first, middle initial)
    • Level of student
    • Date of birth
    • Email address
    • Expected date of graduation
    • Total number of onsite clinical hours per student
  • Step 3: Complete the precepted placement request form. You will receive a confirmation email when we receive the request.
  • Step 4: You will receive another email confirmation once leadership has approved or declined your placement request. (at least 5 weeks prior to your clinical start date).

Important Notes:

Aurora Health Care may assist in coordinating preceptors for medical assistant, undergraduate RN*, accelerated BSN*, and accelerated MSN students.

RN to BSN, MSN, CNS, CNL, and DNP students need to select their own preceptors.**

*For undergraduate RN and accelerated BSN students, please be aware of the important deadlines for submission.

**For students selecting their own preceptor, please allow for 2 months from initial request to clinical start.

Steps for Precepted Students

Welcome accepted precepted students! We are excited to have you as a part of the program.

On the first day of your clinical experience, you will need to complete the Student and Instructor Confidentiality Agreement & Orientation Checklist. This form must be completed before Epic access will be granted.

DNP (Doctor of Nursing Practice) Information

If you are a Doctor of Nursing Practice (DNP) student with a research project, please review the requirements on the Nursing Research page.
