End of Life & Hospice Care

Compassionate Care for the Whole Family

No matter who we are or where we come from, we all want the same thing as we near the end of life: comfort, dignity and peace. The Aurora at Home Hospice Program can help you achieve these fundamental goals. Our team of caring and experienced specialists will work with you and your family during this transitional period, ensuring you’re on the right path to fulfill your end of life needs and wishes.

Hospice is a concept of care that puts your needs and comfort first. It recognizes that a life-limiting disease affects you, your family and every aspect of your life. It also acknowledges death as a natural part of life, and can help you and family members mentally and spiritually navigate your final months. Because of its comprehensive and supportive nature, hospice is something to consider before you are confined to a bed or unable to communicate.

What to Expect

What is hospice care? Unlike palliative care, hospice care is intended for patients at the end of life. Hospice care may be right for you if you have been diagnosed with a terminal or life-limiting condition and you wish to understand all of your options.

People who have experienced a stroke or have been diagnosed with cancer, renal disease, pulmonary disease or heart disease may want to consider hospice care.

Talk to your doctors and caregivers about the best options for you.

The Aurora at Home Hospice Program offers a comprehensive range of services, including:

  • Medical care
  • Pain management
  • Home hospice care
  • Infusion Therapy
  • Nutritional counseling
  • Physical therapy
  • Spiritual services

Our hospice program is designed to help adults and children with a variety of conditions. We'll work with you in your home or a nursing home, assisted living center or hospital.

Hospice care is also available at certain Aurora Medical Centers and the Aurora Zilber Family Hospice.

Admission to hospice is based on need, rather than ability to pay. Once you enroll in the Aurora at Home Hospice Program, we will contact your insurance company to determine your hospice benefits.

Medicare and Medicaid cover all costs related to hospice care, including visits from team members, equipment, supplies, medications and comfort-related therapies. Private insurance policies vary.

If hospice coverage is not available, we can connect you with financial counseling services.
