  • Fill out paperwork. Even if you have been coming to us for years, we may need you to fill out new paperwork if your health insurance has changed or if you need to confirm your personal information.
  • Talk about your lifestyle, including diet and exercise habits. We may also ask about your sleep routine.
  • Discuss changes to your health history or the health history of a close family member, such as parents or siblings.
  • Share information about major life events such as a new job, divorce or other significant changes. Our providers will want to know how the changes are affecting your daily life.
