For all Community Health Surveys, respondents were scientifically selected so that surveys would be representative of all city or county adults 18 years old and older. The sample of random telephone numbers included both listed and unlisted numbers. Respondents within each household were randomly selected using the Hagen/Collier selection technique. At least 8 attempts were made to contact a respondent at each household. Screener questions verifying location were included.
Data collection was conducted by Management Decisions Incorporated, and surveys were conducted by JKV Research, LLC.
