Application Overview

We accept application forms October – December each year. All applications must be postmarked by December 31st. Applications postmarked after this date not will be accepted.

A $50.00 non-refundable application fee is required. Submit a personal check or money order payable to Aurora St. Luke’s Medical Center.

Application Requirements 

Class size is limited, and not all applicants are admitted to the program. Applicants who are selected for interviews will be notified by email.

You must meet all of the following application requirements. If you don’t, you will not be eligible for admission to the program.

All applicants must:

  1. Meet the program’s personal characteristic requirements.
  2. Comply with one of the program’s 3 prerequisite pathways. We encourage you to complete all prerequisites by the end of the enrollment period.
    1. Pathway 1 applicants must complete their allied health program prior to the start of the sonography program in the fall.
    2. Pathway 2 applicants must complete all program prerequisites prior to application.
    3. Pathway 3 applicants must complete all prerequisites and be in good standing prior to the start of the program.
    Applicants must achieve a minimum GPA on mandatory coursework as defined by the program.
  3. Complete a job shadowing experience.
    1. Each candidate will complete a minimum of four hours in cardiac AND a minimum of four hours in general/vascular.
    2. Acceptable sites for a job shadowing experience are a hospital or medical center.
    3. Submit a completed job shadow verification form with your application.
  4. Provide two professional references, including the reference checklist, along with a formal written letter. College professors, health care professionals and current managers or supervisors are considered appropriate professional references.
  5. Have current, direct patient care experience. You must have the Verification of Patient Care form completed by your current employer and return the form with your application materials. Failure to submit the required documentation will result in an incomplete application.
    1. Pathway 1 applicants must have a minimum of 200 hours of direct patient care experience. Experience may be within a current health care career, or clinical experience (not including proficiency or clinical lab demonstration hours) gained while attending an allied health education program.
    2. Pathway 2 and 3 applicants must complete a certified nursing course and have a minimum of 100 hours of direct patient care, in addition to the total hours for the CNA course. Current direct patient care experience as a certified nursing assistant (CNA) is obtained in either a hospital or nursing home setting.
  6. Submit a brief autobiography covering the past four years of your life. It should state why you have an interest in the field of sonography and Aurora St. Luke's Medical Center School of Diagnostic Medical Sonography.

Submitting Transcripts

Official academic transcripts from post-secondary educational institutions are required to document completed prerequisites.

To maintain eligibility, your official, sealed academic transcripts must be received and postmarked by the application deadline. Transcripts should be sent directly to our program by your educational institution.

Faxed, emailed or hand-delivered applications will not be accepted.

General Eligibility 

All required information must be submitted by the due date. If you miss a deadline, your application will be rejected. Official transcripts must verify all educational information provided. Any discrepancies will terminate your eligibility.

Applicants considered for the program will be required to pass a drug screen and physical exam.
