Fellows spend two months on inpatient palliative care service at this community-based setting. Home to Aurora's Internal Medicine and Geriatric Medicine programs, the teaching and learning opportunities at Aurora Sinai Medical Center are plentiful.

While on this service, fellows will work with patients with complex psychosocial situations and limited resources. Fellows will have ample exposure to patients with dementia and those presenting from a long-term care setting. Fellows will also work with their colleagues to evaluate patients for hospice care.

Fellows spend four months on inpatient palliative care service at this quaternary care setting. One of the busiest hospitals in the greater Milwaukee area, Aurora St. Luke's Medical Center provides fellows with volume, diversity and complexity of patients. Fellows will be exposed to critically ill patients from the five different ICUs within the campus.

In addition to working with the primary care team, fellows will collaborate with oncologists, cardiologists, pulmonologists and other subspecialists. Throughout these four months, fellows will participate in interdisciplinary rounds, which aim to coordinate patient care and streamline patient transitions across the continuum. Additionally, fellows will deliver formal and informal presentations to the palliative care team and participate in the Ethics Committee.

Inpatient hospice

Aurora Zilber Family Hospice

Fellows spend about two and a half months at this 18-bed, not-for-profit hospice facility. Aurora Zilber Family Hospice is an integral site for training medical students, residents, fellows and APPs in palliative care. Interdisciplinary team members include hospice physicians, APPs, RNs, LPNs, social workers, chaplains, pharmacists, volunteers and bereavement counselors.

Throughout service, fellows will develop skills in assessing and treating hospice patients in need of inpatient care (home hospice visits, via Aurora Health at Home, will also be conducted when on this service). Fellows will learn to manage end-of-life symptoms, address goals of care, and identify and communicate signs of impeding death.

Fellows will also participate in interdisciplinary team meetings where they will become familiar with the role and functions of the medical director and other members on their team, as well as become familiar with Medicare hospice regulations.

Geriatrics

Fellows spend one month working alongside geriatricians, learning the nuances of palliative care needs as they pertain to the elderly, such as the progression and management of neurological illness (dementia, ALS, Parkinson's, etc.); chronic, non-cancer pain; wound care; and geriatric syndromes (incontinence, delirium, non-cancer-related functional decline, frailty, etc.). This portion of the program will take place in both inpatient and outpatient settings.

Pediatric palliative care

Fellows spend about two weeks in the ambulatory and/or inpatient pediatric palliative care setting/NICU. Alongside the palliative care team, fellows will have an opportunity to participate in the care of children with chronic illnesses and/or those who are at the end of life by providing symptom management, assisting with complex decision-making and goals-of-care discussions, facilitating family meetings and offering support.

Consultations with pediatric oncology are a large focus of this rotation.
